• IBP National Certificate of Good Standing
• IBP National Certificate of No Pending Case
• Office of the Bar Confidant Certificate of Pending Case and Clearance
• IBP Local Certificate of Good Standing and No Pending Case
• NBI Clearance
• Profession Tax Receipt for the current year and cedula
• Three Specimen Signature
• Pictures of your office
• Dry Seal Impression
• Three Passport Size photograph with signature
• Affidavit of Publication and Copy of the Official Publication
